I have a question. We have an employee who is adopting a child from another country. What does the employee fill out for adoption? The HealthCare Certificate only asks medical questions if an employee or family member is ill.

An employee who is adopting a child fills out the same FMLA forms as a father would fill out when taking time off to care for and bond with a newborn.

You would have the employee fill out form WH-381 from the U.S. Department of Labor, or a similar form. This form is used both to request FMLA and to notify the employee of FMLA rights as required by law. You are correct that no certification form for a serious health condition is required, because no one in the family is sick.
