i need to make form with employee name and i need to know each one of my employees is going in car and how he spend the time using the company car with the following:

1-personal use
2-emergency cases
3- weekend use
etc..
your urgent reply

What you actually need is a mileage log for each company car. The employee will record the starting and ending mileage for each trip, with a code for the type of usage (personal use, emergency call, weekend use, etc.) and a space for details about that event. You may also want to have the employee log the start and ending times and date.
