Gross Wages/Vehicle Allotment

Hello. I am filling out a form from the Department of Labor and they are asking for a previous employee’s gross wages earned. This employee was paid a bi-weekly salary plus a $500 vehicle allotment bi-weekly. Do I include the $500 in the gross wages? So, if she made $2000 plus her $550 for the week ending 4/1/17 would I enter $2500 or $2000?

Usually, DOL forms are looking for the total amount paid to an employee. Thus, if the employee made $2,000 as her salary plus $500 for her vehicle allotment, then she was paid a total of $2,500. That is what should be noted.
